Rehabilitation & Health of Older People (ARHOP)
Assessment Treatment & Rehabilitation
(AT&R)
This service provides assessment, treatment and rehabilitation for people 65 years and over. It focuses on the needs of those who have multiple medical problems and functional impairment,
who require specialised multi-disciplinary assessments and treatment, such as:
- Recent acute or semi acute illness or an exacerbation of chronic illness
- Non-specific illness presentation with falls, confusion, immobility, and incontinence
- Rehabilitation needs following medical illness, stroke, surgery or fractures
- A need for comprehensive assessment to ascertain their future care needs and maximise functional competence
The
service is provided by multi-disciplinary teams consisting of
Geriatricians, Nurses, Occupational Therapists, Physiotherapists, Speech
Language Therapists, Social Workers, Dietitians, and Pharmacists.
Decisions around care options are made with the client and their
families/whaanau.
The service provides Inpatient Care in Wards 22a, 23, and 24. Patients
may also be seen at home, in rest homes or private hospitals for:
* Medical opinion and advice on management, further investigations or
rehabilitation
* Advice on existing level of care and support
* Advice on need for institutional care and level of care required
Outpatient clinics are also located at AT&R Unit in Middlemore Hospital,
Manukau SuperClinic™, Pukekohe Hospital, Waiuku Hospital, and
Botany Downs SuperClinic™.
