KEEPING YOU INVOLVED Introduction
After this outpatient
appointment, a letter about you and your health will be sent to
your GP. By copying this letter to you, we hope to keep you
informed so you can understand your heath and treatment better.
The key aim of this
pilot is to keep patients fully informed about and
involved in their treatment. It also gives you the
opportunity to correct any inaccuracies in the
clinician’s correspondence. Copying letters to patients
is considered very good practice by many and Counties
Manukau DHB are looking to see if it meets your needs.
Yes, you can. But you
would need to request it through our clinical records department
which is time consuming for you and also for our staff.
If you have difficulty
understanding the medical terms or have any other questions,
make an appointment to discuss it with your GP.
It is your
responsibility to ensure that we have your correct address. If
there is someone else in your home with the same name or
initials, please let us know how you would like the
letter addressed.
This pilot is
one of our many programs to improve services provided to
our patients. We are requesting you to complete the
survey (sent along with the outpatient clinic letter)
so:

you along with the letter.
(The survey
takes only 5 mins to complete. Please return it to us in
the pre-stamped envelope provided.)
You are
assured complete confidentiality. No individual
identifiable information will be given to clinical
departments. The identification number on the envelope
is for mailing purposes only. This enables us to check
your name off the mailing list when your completed
survey is returned.
