We have developed a number of Advance Care Planning
documents including an Advance Care Planning information booklet
for patients and an
Advance Care
Plan where patients can record their views on care and
treatment.
All of these tools are currently being
used across all secondary care settings where Advance Care Planning has
been introduced.
We have developed information
systems that enable a person's Advance Care Plan to be stored electronically
on the patient's file at the DHB and to be accessible to health
providers involved in that person’s care.
Following the
implementation of Advance Care Planning in secondary care we will
evaluate the effectiveness of these documents and processes that have
been developed.
This information will advise us on the resources we require to build our
infrastructure to sustain Advance Care Planning in the future.
We have appointed an Advance Care Planning
Facilitator who will support and mentor health teams to carry out
Advance Care Planning where it has been introduced.
